I agree with one of the previous reviews. It doesn't sit down on top of the shoulder. However, it works well. It empties and fills easily. I had the knee sleeve and thus is a great addition. So much better than ice packs.
The cuff is good but would be great if it fit well. The upper 1/4 of the cuff doesn’t rest against my shoulder. It sits about a half an inch above that portion of my shoulder, and that distance increases toward the very end of the cup. It’s sort of fans out a bit, and off my shoulder. One of the issues that consumers should be aware of is that if you open any product that you order from this company, you cannot return it. Even if you have not used it, and simply opened it to ensure that all the pieces were there and working, you cannot return it. They refuse to accept any returns for any products that are opened, or any cold systems at all. I would recommend buying this product from a company with a typical, return policy. Otherwise, you may find yourself stuck with a $200 medical device that doesn’t quite work the way you anticipated it would based on the product description.

Terrible customer service . Shoulder cuff never sat comfortably
We are using the CryoCast system for the wifeys recovery from knee surgery. Wonderfully simple system that provides cold therapy and a little bit of compression. Sometimes too much compression according to the wifey. One thing we both are disappointed with is that the CryoCast Cuffs do not provide cold therapy all the way around the affected area. The knee cuff only rests on the front of the knee with a nice space for the knee cap to fit. Cools all around the frond of the knee. The swelling on the back of the knee needs attention, so I purchased the CryoCast Thigh Cuff. Images of this cuff appeared to have it wrapping around the entire thigh. This is not the case. The cold therapy and compression is on the front and sides only. Straps wrap around the back of the thigh. Not sure it would make a difference, or complicate things, but what if the cuff was called a 'Quadricep or Hamstring Cuff' for the front or back of the thigh? Ah well, I'm sure that wouldn't satisfy everyone either. Regardless, the system is wonderful. I'm considering getting the 'Back/Ribs Cuff'.
